AF Occupational Therapy services are by appointment only, please read on for more information.

I am registered as an Occupational Therapist and an Occupational Therapy Australia (OTA) accredited Mental Health Occupational Therapist for Medicare Psychological Services (Mental Health Care Plan).
You can obtain rebates from Medicare for the following services:

1. Chronic Disease Management Plan – Occupational Therapy MBS 10958
2. Mental Health Care plan – Focused Psychological Strategies (FPS)-MBS 80145

   I also complete assessments including:
   a. Functional Capacity Assessment and Service Recommendation
   b. Cognitive Screening and Assessments (Age 18+)
   c. Sensory Matrix and profile assessment (https://sensoryintelligence.com/self-assessments/sensory-matrix/)

   c. Home-based functional assessment (excluding home modification)
